When creating a defect from an execution, the issues are created before clicking on the tick button

A true "edit mode" wouldn't create the defect until the tick was clicked, as that would be consistent from a transaction point of view.

The current method causes problems with testers believing they've created their defect and forgetting to click the tick - which does not link the issues, leaves the step devoid of the defect key, and leaves the defect orphaned.
