Actual result evidence are duplicated when defect is created from a test run step

Description
when adding an attachment to the actual result step field and creating a defect from the step with 'Include Steps Table in Description' enabled, the attachment is copied twice to the defect

Pre-conditions
 Have Include Steps Table in Description enabled in global/project settings

How to reproduce

1. Create a test execution for a manual test
2. Open the test run 
3. Choose a Step and expand the actual result field
4. Add an attachment to it and click save
5. Create a defect on that step
6. Open the created defect issue

Actual result/Attachments

We can observe the attachment/evidence added in the Actual Result field is duplicated in the attachments of the defect issue. (see replication in the attachments)

Only one attachment should show

Not adding the attachment in the Actual Result Field, only in the evidence.
