Duplicated entry in history when deleting a Test from a Test Plan.

Steps to reproduce:

1. Create a new Test
2. Create a new Test Plans
3. In the Test issue page, add the Test Plan using the button "Associate Test Plans"
4. Still in the Test page, remove the Test Plan using the "More Options" button ( [...] )
5. Check the history where it will report the removal of the Test Plan twice (please check screenshot in attachment)
