When using the add-on Default values for Create Issue Screen, when raising a Defect from a Test Run, the template configurated for the issue does not appear on the screen

Description

When using the add-on Default values for Create Issue Screen (https://marketplace.atlassian.com/apps/1211873/default-values-for-create-issue-screen?hosting=server&tab=overview) and define a template for the issuetype Bug, when raising a Defect from a Test Run, the template does not appear on the screen.

It usually worked for later versions as 4.1.5.

1. Define a template for the issue type Bug on the configurations of the add-on:

2. Go to an Execution Details page and raise a Defect.

The template set does not appear:

Expected result

It should appear as when we click on the Create button from Jira.
