Unable to add multiple Xray attachments to issue

Description
Unable to add multiple attachments to issue when creating a defect from a step.

Pre-conditions

How to reproduce

1. Create a test
2. Add 25 steps with an image in the field (action, data or result)
3. Create a test run with that test
4. Create a defect in the step 25
5. Check the defect you created

Actual result/Attachments

The defect description has some missing images

Expected result

All the images should be available

Notes:
If you can't replicate the problem, try more steps.
