Inconsistent behavior of included attachments when creating defects from the execution screen

Description

There is an inconsistent behavior when creating defects from the execution screen. Sometimes, all attachments are added to the created defect, and other times a dialog appears to the user when so that he can choose the attachments to add.

Steps to Reproduce

1. Create a manual test with 3 steps 
2. Go to the execution screen
3. Add a file to step 1
4. Add a file to step 2
5. Add a file to step 3
6. Create a defect in step 3.

Actual Result / Attachments
The attachments selection dialog appears to the user with just a single attachment (created in step 3).

Expected Result
The attachments selection dialog appears to the user with all attachment until the step where the defect is being created.
